Below you will find links to two recent Survey & Certification Memos. SC-15-31 pertains to the 2014 Focused Dementia Care Survey, as well as plans for expansion efforts in 2015. SC-15-34 is in reference to a civil money penalty grant award that will expand upon the work of the National Partnership.
Alliant Quality, the Medicare Quality Innovation Network - Quality Improvement Organization (QIN-QIO) for Georgia and North Carolina, is inviting all nursing homes to participate in “SPACE” (Southern Partners Action Collaborative for Excellence), a learning and action network designed to focus on use of quality and performance improvement practices. The practices are designed to focus on person-centered care, share best practices and learn from one another while continually using data to drive care improvement. Opportunities for improvement locally, regionally and within the state will be identified based on state and regional data. “SPACE” will coordinate and integrate with other Learning and Action Networks (LANs) operating in Georgia and North Carolina to reduce Healthcare Associated Infections and to improve Transitions of Care whenever possible. Additionally, the Collaborative will align and coordinate efforts with partners and stakeholders to avoid and eliminate duplication of efforts throughout the contract.
